Job Description:

A company is looking for a Clinical Pharmacist to engage with Medicare members and healthcare providers for optimal medication management.

Key Responsibilities

Conduct outreach to Medicare members to improve medication adherence
Complete Comprehensive Medication Reviews (CMRs) and provide counseling on medications
Collaborate with pharmacies and providers to address adherence and SUPD gaps

Required Qualifications

Bachelor's degree or advanced degree in Pharmacy
3+ years of experience in a related field
Current active licensure as a Pharmacist in the State of North Carolina
Availability to work from 10:00 AM to 7:00 PM, Monday through Friday
